Click the Download button, then click Open. Then use File...Save As to save the form on your computer. The first page of the form has fields that you fill in and then tab to get to the next one. The second and third pages you can just type in the text as needed, but be sure to tab between the columns. You may have to delete rows to maintain the spacing on the page.
